Forms4Travel was developed by single mom and New York attorney Linda Kagan to help give parents peace of mind when they travel or when their children are traveling. In light of the recent international custody battle between a US father and a Brazilian step-father, many parents may be asking themselves how they can protect their children when traveling.
Forms4Travel covers a variety of travel situations including domestic & international travel, babysitter forms, and traveling parent forms. These forms answer questions like “Who can pick your child up from the airport” or “What medicines can be administered,” and provide specific instructions on schedules and travel allowances.
Most parents leave emergency numbers and allergies with babysitters when they go out for a night or communicate individually with organizations when sending their children aboard - but what makes Forms4Travel different. I asked this question to Linda Kagan and she answered,
“The difference between our forms (especially the affidavit versions) and a letter is that they are more comprehensive and by notarizing a parent’s signature, they are easily recognizable to officials and this formality can be very important in emergency situations.”
Linda clarifies that even those most of these forms are notarized, they “…are not contacts per se, but directives about care and travel… acknowledge the travel parameters… is very helpful for parents to identify the limitation of travel destination and dates in a clear way.”
All forms can be purchased online and the price varies depending on the form, but the price ranges from around $13 USD to $30 USD.
